Contact us by phone, email, or by using Contact Us or Request Appointment forms to schedule an appointment.

To Purchase a Gift Certificate:
1) Make sure recipient lives within our service area.
2) Use the form below to order the certificate.
3) Complete purchase by making payment.
We will send the certificate to the designated recipient upon receipt of payment. Thank you for your order!

Terms:
1) Unused balances may be used towards the purchase of additional visits.
2) If the cost of a visit is more than the unused balance, then the recipient is responsible for paying the difference.
3) Certificates cannot be redeemed for cash.
4) Certificates issued by email are not transferrable.
5) Certificates must be presented at the time of the appointment.
